• 1. I hereby apply as TFD independent members / retailers. I understand that upon registratrion of membership, I will enjoy benefits and discounts provided by the company.


• 2. I hereby agree that I am entitled to all rebates, commissions and other incentives provided that i maintain monthly personal purchase that maybe required by the company. All rebates and commissions are subject to a 10% withholding tax.


• 3. That any changes or revisions on the membership application form shall only be allowed upon submission of a written request and that all changes shall be subject for approval.


• 4. That I agree to comply with company procedures of which FFI reserves the right to amend as it deems fit. Any such amendments shall immediately be implemented to all members, retailers and distributors.


• 5. Membership fee is NON-REFUNDABLE

FILTREPRENEUR FRANCHISE, INC., reserves the right to terminate and forfeit commissions, bonuses and privileges on present or future members and distributors found to be in violation of company rules and regulations.
